Below are links to forms and applications that are available in the Auditor's Office.  Forms and applications can be returned to the Auditor's Office during regular business hours.

Auditorium Rental Contract - Please call the City Office at 843-7828 to reserve your date or for more information.

Building Permit Application - Return application to the Auditor's Office for approval by the building inspector.

Building Permit Fee Schedule - Fees are determined by the building inspecdtor.

Variance or Appeal Application - Must be approved by P&Z and New Salem City Council.

Gaming Permit Application $25 - Must be approved by the City Council.  Meetings are held the second Monday of each month.

Liquor License Transfer Special Permit Application $25 - Must be approved by the New Salem City Council.  Meetings are the second Monday of each Month.
